What documents are proof of health insurance?

In connection with the introduction of the system of electronic verification of beneficiaries' entitlements (eWUŚ), the verification of patients' entitlements is carried out each time before the provision of services on the basis of the PESEL number. In the case of a child under 3 months of age who may not yet have their own PESEL number, the right to benefits is confirmed by the PESEL number of the child's parent or guardian.

If there is no declaration in the system, the patient, if he confirms his insurance, will have the opportunity to fill in a statement of his entitlement to health care services.

On this basis, a patient may be admitted free of charge on a given day for a service contracted with the National Health Service or have a prescription issued for a reimbursed commercial visit.
If I forget my insurance document and I go to a primary care doctor or a clinic within the National Health Fund, will I be accepted?

Since 01.01.2013 in connection with the introduction of the system of electronic verification of rights of beneficiaries (eWUŚ), the verification of patients' rights is carried out each time before the service on the basis of the PESEL number. In the registration, the patient presents an identity document (identity card, passport, driving licence, in the case of persons studying up to 18 years of age, a school ID card and a biometric ID card in the case of children) together with their PESEL number.

Is it possible for a doctor on a private visit to make a referral for laboratory/diagnostic tests, rehabilitation, to an outpatient clinic or to a hospital under the National Health Fund?

Within the framework of commercial treatment, a doctor cannot issue referrals for examinations refunded by the National Health Fund. Such referrals may be issued only in the case of treatment in clinics and outpatient clinics which have signed an agreement with a voivodeship department of the National Health Fund in a given calendar year. In most cases, a referral issued by a family doctor is required for specialist treatment. A referral for specialist examination and rehabilitation can be made only by a specialist of the outpatient clinic of the National Health Fund on the basis of the patient's documented history of treatment in that clinic.

In life-threatening or health-threatening situations, a doctor may make a referral for hospital treatment.
What should I do if the eWUŚ system does not confirm my insurance?

If there is no notification in the system, the patient has the possibility to fill in a statement of the patient's right to health care services, on the basis of which the patient can be admitted on a given day free of charge for a service contracted with the NFZ.

In such a situation, the patient should verify his insurance with the payer of contributions as soon as possible and, with this document, report to the appropriate branch of the NFZ, where he should explain the lack of notification in the eWUŚ system.
